Becoming a patient

From your first visit at our fertility center to your final treatment, we aim to provide an excellent experience and treat you like family. We will help you build a custom treatment plan to help you reach your goal of parenthood. Getting started is as simple as calling our office or requesting an appointment online, or having your primary doctor refer you.

For new patients

Before your first appointment, one of our licensed nurses will do an initial phone consultation with you to complete your patient registration and gather information about your attempts to conceive, your medical history (both yours and your partner’s), and special goals or questions you may have.

Appointment information

When you arrive for your first appointment, a staff member will greet you and check you in. Most of your paperwork should be completed before you arrive, so there will only be a few forms to complete. We may further discuss your medical, reproductive and family history, as well as review lifestyle factors. We want to know and understand you to give you the best care we can.

Bringing any relevant medical records, prior fertility evaluations and treatment reports to your first appointment is helpful. Though you don’t have to postpone an appointment waiting for records, if you’ve had any of the below tests, bringing these results to your visit can make it easier to have a clearer picture of your fertility:

  1. Previous chlamydia and gonorrhea testing, which you typically will need to do if not done previously, as testing and treatment could spread these infections
  2. Last Pap smear
  3. Hemoglobin or hematocrit (are you anemic?)
  4. TSH (is your thyroid working?)
  5. Previous testing for PCOS or recurrent miscarriages
  6. Hysterosalpingogram (HSG) images (preferable to just a written report)
  7. Operative notes (particularly for laparoscopy or a tubal ligation).

Once we’ve reviewed your history, we can discuss why conception has taken longer than it should for you. We will outline our plan for an infertility evaluation and explain the reasoning for each test, as well as a timeline for testing and beginning treatment.

We will always make sure that you leave your first visit with a clear understanding of the treatment plan that we have decided on together and the next steps for you as a patient. Always tell us if you have any questions so we can talk through them as soon as possible.

Throughout the entirety of your care with us, we aim to make the process and communication easy to understand so that together we can help you achieve your fertility goals.

Appointment checklist

Please bring the following to your appointment:

  • Insurance and identification cards
  • A list of all medications you are currently taking (including over-the-counter remedies, vitamins and herbal supplements)
  • All medical records, including prior fertility evaluations and treatment records
  • Write down any questions or concerns you may wish to discuss
  • We recommend couples come to appointments together. However, if you would like, you can bring a family member or friend. That person can help by listening to your doctor’s comments and responses to your questions and writing down important information for you.

Health insurance coverage for fertility treatments

At this point, insurance coverage for infertility is often poor or nonexistent. At multiple points, we’ve had patients thinking they have coverage, only to be denied by insurance. Sometimes even if patients are permitted copays and deductibles, they made minimal difference from not having insurance.

As a result, like many dental and dermatology practices, we don’t directly accept insurance. We keep our prices low, as though you had coverage for fertility, and then are happy to work with you to submit an out-of-network benefit claim to your insurer. If you have questions about billing or how to do this for testing, IUI (intrauterine insemination) packages or IVF (in vitro fertilization), please contact us.
